=== Mechanisms and therapeutics for neurodevelopmental disorders === Sur’s group has applied this understanding of plasticity to study disorders of brain development. Rett Syndrome is a devastating neurodevelopmental disorder caused by mutations in MECP2, a transcriptional regulator. Sur hypothesized that a core mechanism of Rett Syndrome is the persistence of immature synapses which may be induced to mature. Rett model mice indeed have a deficit in PI3K/Akt/Erk signaling and PSD95 expression, leading to immature excitatory synapses and prolonged visual cortex plasticity that extends into adulthood. Application of IGF1(1-3) peptide, which degrades IGF binding proteins produced by astrocytes and enhances IGF1 availability, and of full-length IGF1, upregulates these signals to restore normal synaptic plasticity and function and improve behavioral phenotypes. Human Rett IPSC-derived neurons show deficits in IGF1 and similar effectiveness of IGF1 in restoring PI3K, AKT and S6 signals. Based on the lab’s work, an IGF1(1-3) mimetic – trofinetide - was employed in clinical trials for Rett Syndrome. In 2023, trofinetide was approved by the FDA as the first treatment for Rett Syndrome. ==== Dependence and withdrawal ==== Tolerance is expected to develop with regular methamphetamine use and, when used recreationally, this tolerance develops rapidly. In dependent users, withdrawal symptoms are positively correlated with the level of drug tolerance. Depression from methamphetamine withdrawal lasts longer and is more severe than that of cocaine withdrawal. According to the current Cochrane review on drug dependence and withdrawal in recreational users of methamphetamine, "when chronic heavy users abruptly discontinue [methamphetamine] use, many report a time-limited withdrawal syndrome that occurs within 24 hours of their last dose". Withdrawal symptoms in chronic, high-dose users are frequent, occurring in up to 87.6% of cases, and persist for three to four weeks with a marked "crash" phase occurring during the first week. Methamphetamine withdrawal symptoms can include anxiety, drug craving, dysphoric mood, fatigue, increased appetite, increased movement or decreased movement, lack of motivation, sleeplessness or sleepiness, and vivid or lucid dreams. Methamphetamine that is present in a mother's bloodstream can pass through the placenta to a fetus and be secreted into breast milk. Infants born to methamphetamine-abusing mothers may experience a neonatal withdrawal syndrome, with symptoms involving of abnormal sleep patterns, poor feeding, tremors, and hypertonia. === Pharmacodynamics === Benorterone is an antiandrogen, or an antagonist of the androgen receptor (AR), the biological target of the androgen sex hormones testosterone and dihydrotestosterone. In one study, the affinity of benorterone for the AR was found to be about 5-fold greater than that of cyproterone acetate in rat prostate cytosol; the Ki values were 0.7 nM for benorterone and 3.7 nM for cyproterone acetate, which were 243% and 46% of those of testosterone (Ki = 1.7 nM), respectively. However, another study found that benorterone had only 11% of the affinity of dihydrotestosterone for the androgen receptor. Although an antiandrogen, benorterone actually is a very weak partial agonist of the AR and has been reported to possess weak androgenic activity. The same is true for cyproterone acetate and other steroidal antiandrogens. Unlike certain other steroidal antiandrogens such as cyproterone acetate, benorterone is not also a progestogen, instead being described as a selective and pure AR antagonist similarly to nonsteroidal antiandrogens such as flutamide and bicalutamide. However, although it is described as not being a progestogen, benorterone was found to produce "a highly variable decrease in plasma testosterone levels," indicating that it has weak antigonadotropic effects. The reasons for this are unclear, as other pure antiandrogens such as cyproterone (not cyproterone acetate) and flutamide do not do this and instead produce consistent increases in testosterone levels.
